Is it difficult to maintain rankings for a health blog ?

I used to write content for a health related website. He was maintaining very good quality content on his website and ranking well also for many keywords. But over time, he has stopped ordering from me and I see no content being published on the site. This project of his is like abandoned. Also, if I search for any medication related articles, countful authority sites dominate for all keywords. So is it not possible for small publisher's to run and rank health related blogs now ?

Currently not in the heath niche myself but I hear all the time that you need strong established EAT. Which makes sense since it's YMYL niche.
 
Health has become more of a big-boys game recently.

So many factors involved that it's hard for a regular guy to make any breakthrough in terms of ranking and/or profits.

What are other factors except content quality and high expense SEO ?

The health niche is a difficult one, there is also stiff competition from much established websites with several years of experience.

It's like trying to dominate Wikipedia in topics related to biography or information about popular people and entities.

i think this is a benefit for the consumer. Last thing you want to find for a health related issue, to come across an affiliate site. This niche should really be left to well established site with real authority
 
Yes, health is always a competitive niche, it's rankable but it needs a lot of effort and money. Targeting low competition/micro niches can help you to sustain. Or affiliate websites are also a better choice.
