I applied for ******* was refused. I told the manager, I get traffic so
    I mainly promote offers through
PPC and email marketing. I have a list of 5000 emails which I collected in the past and

they work really well with some of the offers. I also drive
traffic through social networking sites like facebook and twitter.

But the manager gave me the following reasons for refusal.
1、 You did not provide enough relevant information on your application.
2、 We do not accept cash incentive / GPT traffic (end users completing surveys for real

    money).

How can I change, to obtain the manager's consent, thank you.

I think you even didn't add a website for the application.

Honestly you may want to lie during the application as long as you know enough about the other advertising mediums to sound like you might do them, and maybe even up what you say what you make monthly (its not like they'll know if you're lying, most of us do business with multiple networks at once so it just means that we're not dedicating it all to their network when you don't pull it off with them).

I personally never put in a website and I've never been denied from a network, but I do always emphasize that I am doing social (can do high volume) and am open to/actively expanding my sales model and mediums. Definitely don't up play incent traffic, almost no one has a lot of offers for that and it gives them a lot of trouble quite often.

You could also get some one to refer you.

For CPA companies, I always explain how I've been successfull at Affiliate marketing, so I want to see if I can achieve the same success using CPA.

Sounds like CPA Lead? I can't tell because of the asterisks. But, I think it was just an autoresponder, since you probably didn't enter a website. This happened to me as well, and I think you should re-apply but with a blogspot/wordpress blog, along with your other methods of marketing.
