How to get back into a CPA network once banned?

How do you get back into a CPA network once you get banned and you live in the UK? Would a simple business with a trading as name and business bank account work? Okay well the address and telephone number isn't much of a problem when it comes to applying again, but how about the first and last name when applying to an affiliate network? How do you sort that one out?

Please read the thread again and compare it to my thread you may notice the difference between me being in the UK, asking for advice in the UK and the thread you posted being in the US where completely different stuff apply.
