How to do SEO for 'California XYZ' when searchers only search for 'XYZ'?

I'm doing SEO for a health insurance agent, and brought his website on the 3rd position on Google for 'Medicare Advantage' (and many other competitive keywords) - so I'm pretty happy, but....

...bad thing is that he is only licensed in California... So from 100 inquiries he gets, only a handful are from CA people.

Here is the problem: Nobody looking for a Medicare advantage insurance plan searches for 'California Medicare advantage', so getting a high Google ranking for this keyword won't work.

So I'm twisting my mind how to get him more CA prospects...

Anybody of you has a similar problem and a hint, a solution, or anything that would get me thinking outside of the box? May be I'm missing the very obvious...

PS: I know we should be selling the out-of-state leads, but that's not the point. How to get only CA searchers with organic traffic, when they don't search for 'CA' is what I'd like to accomplish...

You could try adding something like "Medicare advantage for california residents" in your description or title. People who do not live in California are going to be more inclined to pass it up.

He explains how to get the box of 10 local listings to show up in Google. Unfortunately it does not show up for every keyword so you have to find the keywords that will give the box.
 
Do you do PPC for him? Google Adwords can produce highly targeted results in specific cities. So, for example, you could set up keywords such as "Medicare Advantage" from only those searchers in San Jose (or whatever surrounding cities that he markets to) in your adwords account and just bill him double what your click charges are. Probably a lot quicker and more targeted than SEO.
 
A trick would be ( i am not 100% sure, whether it is possible for California), but to use google webmaster tools and specify there the location of the site. This improves your local search results
 
For starters, according to the Gxxgle Adwxrds tool, "california medicare advantage" was searched for 1300 times last month.

So, in essence, just add California in the keyword phrase. If you optimize for California Medicare Advantage, then you have an optimized page for all combinations of that phrase: california advantage, california medicare, etc....

Also you can add the code:

on the about us page or contact us page. Google uses this anchor to determine the location of the company for local searches.

also since a small percentage of each keyword is ca surfers you need to develop a huge keyword list to catch the most ca surfers out there. easier t go with local/maps and take it to bing n yahoo.
