Don't know about tools that autogenerate this. With minimal coding experience I would style a html form to look like this and submit the info to a database. If you are running for example a wordpress site, im sure you can find a plugin that generates popups and a plugin that saves a form to a database.

I own this and it will do that, it’s crazy the options it has
