How do i put this on my website ?

Hi guys im going to make a wordpress site with lots of differant videos on it. When a user comes to my site and then clicks the play button on a video how do i get one of these pop ups to appear ? Do i need a script or code or what ?

They would have to fill out the server or whatever i wanted and then they get the movie or whatever it is they want to watch.

Any help would be great
