Clear all your browser settings and cookies and look agin .

If it not detected via a scan might be your browser full of crap

Addon are usally the problam try turning thrm all off or delete them they seem to cause browser problems in some cases ....

Make sure you got no stupid pud secuity flaws on the browser ,scan for security browser for virus problams....

Use ccleaner to help clean the pc

If it is still there then you been hacked and a hacker has added a aflicate link to your form .

Your need to re set your secuity to your host .

This time pick a good hard password ...

Read up on chosing a password for windows .

basically use upper and lower case letters but also use a number and a special character.

    Good luck

Check your plugins, are you using nulled plugins ? try removing the contact plugin and see if the popup appears again.

Also check your wp files for strange codes or install a WP scanner and check your website.

Try installing Adblock Plus coupled with what @redarrow mentioned
