Google Adsense Disabled - Best approach if any to running adsense again?

Back in 07 I was running some of the first tv sites, and although they made me nice boat of cash they got my adsense account suspended. Now 4 years later is there anything I can do to get adsense running again?

What are next best options?

What about tax issues, anything to concerning?

I've had 2 accounts banned; the second was within an hour of them approving me after being banned the first time.

What you should change:

Email Address/Google Account
Address
IP you applied from (preferably from another city, region etc.. not sure proxy would really do it, but I don't know)
Company or Personal account - my first 2 (banned) were company, my latest is personal.
EIN vs. SSN - again i used my EIN for my 2 company (banned) and SSN for my personal (not banned).
Phone # to verify.
Bank account # for deposits.

Only thing I kept consistent was my name, as I figure I can't be the ONLY person in the USA with my first+last name combination.

Also, obviously NEVER put your adsense code on a site that was previously banned.
