Form W8-BEN (for foreign residents and companies)

I am From Pakistan.
I have been wandering here and there to get the solid guide to get this form so i can get a taxpayer i.d
Can anyone help em with that?
I maybe unable to understand what to do.
I would really appreciate any help.

For personal or individual :
Fill point 1 with your full name, point 2 with your country.
Check at point 3 as individual.
Point 4 with your address, point 5 fill with your city, state and post code, then fill your Country in the right side.
Poin 9a enter your Country.
Check box at point 11
Sign, enter the date you write and write your name at Applicant.
