Facebook domain flagging - what triggers it?

    I've only recently gotten back into Facebook and I've now noticed they seem to have a more solid domain flagging system in place. Back when I used to use Facebook for marketing they had the "Are you sure you want to visit this website?" type warning when the user clicked an external link that had been identified as potentially dangerous but it appears that they now require confirmation for any like that links to a flagged domain (or is it page?) and verification is needed when PM'ing another Facebook user a link to a flagged domain.

    I'm wondering if anyone's identified "factors" that lead to a domain being flagged up by Facebook auto-detect system?

    If so, I'd love to hear (and I'm sure many other would) your opinion/examples on what triggers detection :).
