I want to reach maximum site loading speeds and thinking about using W3 Total Cache to accomplish this.
Are there any downsides regarding the plugin?
What is the worst that could happen to my visitors?

Only if you don't set it up correctly.
Losing a kid, getting cancer, losing a loved one. There is actually a lot of bad things that could happen to your website visitors. But again, none of them would be caused by W3 Total Cache unless you don't get it set up correctly.

Each time you update your site, you gotta clean the cache in the plugin section to make sure it reflects on your browser, or you won't be able to see the changes you've made.

That's what I'm worrying about. What about brand new blog posts?

You can set how many minutes before the cache expires and renew, you can also set it to homepage. Personally I set it 6 hours on homepage and 1 day on post.

Actually this is not true. By default the settings are to purge all pages/posts/the blog feed each time they're edited... No need to do that manually. And also logged in users are not served cached pages by default.

This is all editable in the settings.

So can I just "set and forget" W3 Total Cache? And will it speed up my site immediately?

Well right now W3 Total Cache is breaking my website, or at least, it is displaying a broken page for every cache request. No way to fix it and it's not happening on any of my other sites. A few pages were deindexed but they'll come back soon since I refetched my page in WMT.

Damn... looks like W3 Total Cache can also ruin your rankings, huh? :(
