Disawow links or not?

Disawowing doesn't really help, not with algo penalties at least, could work for manual action appeal,
It's more like you're reporting certain websites to Google, you won't actually remove the links' effect,
When it comes to Semrush score, it's not something to rely on,
Probably better than Moz useless spam score, but you'll have to check things manually for anything that could be affecting your website(s)

Yes you should. Diavowing will help a lot with rankings and traffic.

Google confirms that their algorithm is smart enough to identify toxic backlinks, and remove them from the PageRank ranking algorithm.
However, to be more careful, you should also periodically audit toxic backlinks, and disavow them using the Google Disavow Tool.
