Does Clickbank require to fill a W-9 tax form for non US residents?

you don't need to fill a W-9 tax form, you'll get your payment via cheque (after first 5 sales)

You dont need to fill it in, you will receive your cheque when you fit the criteria its something about 3 sales with credit card and something else than you can send it to you bank they will take a little fee from it it was 10EURO or something and then you can enjoy your money :)

If you do over $600 then you need to send it in. You will get paid starting immediately though.

edit - from europe you should be fine. Perhaps a W-8 at some point though as W-9 is for US only. Couldn't say for sure!
