I own a new domain name with 55,000 monthly Google searches in the medical field and $1.15 CPC. It strikes me as hard to rank. Trying to figure out if it is worth the effort. Any comments?

So does the domain name, which I'm assuming is an exact match domain, just get 55,000 monthly searches or are you trying to say the keywords you want to rank the domain for get 55,000 monthly searches total? I'm also going to assume you're using Google's Keyword Planner since you know the CPC, so do you also plan on using PPC advertising to promote this website?

Remember while "anal fissures" might be hard

"explosive anal fissures" is going to be easier.

    "wet anal fissures"
    "public anal fissures"
    "wet explosive anal fissures"

Plently of nice ones if you think outside the box.

The 55,000 monthly searches are for the exact match domain name. I don't yet know how -- or even if -- I will promote the site.

Well, EMDs do not rank nearly as well for their keywords as they did in the past. The CPC only matters if you plan on doing PPC with Google Adwords, so it's irrelevant here. You should search the keyword in Google and analyse your competitors. If your competitors are a bunch of PR2 websites with a low domain authority, low page authority, and only 1 or 2k backlinks, then I would definitely try to promote that website.
