has any1 convert well with surveys. i know about e-mail submit but has anyone get good converts?

Try out cpa*lead . Personally, I would go with email submits, but then from the way you are saying this it will most likely be incentivised, so then you would have to mix in WH traffic, keep your earnings low over many networks, etc.
