I know Adsterra pop under CPM sucks and I don't use it anymore, but their banner ads are great. They offer high CPM for banners. I think you should consider using them.
Alternatively, you can try hilltopads, propeller ads, or adcash. Not sure about admaven, the last time I used them, they're full of malwares and got my sites in troubles especially for chrome users.

None. I built PBN myself thus it's called Private. The best PBN out there is the one that you own and you used yourself.

yes max 3. Sometimes I do two links max just to avoid footprints.
No not native banner. Banner cpm for US traffic is high on my side. Not global CPM.
