I have seen obscene ads that somehow they can still run campaigns without their accounts being banned. The strange thing is that those advertising videos advertise dietary supplements. They use them, but the videos are up to 2 hours long.

The solution for you is to blur the sensitive area, or you can hide it behind another link, with different image content to avoid violating the policy.
 
Of course Facebook will not accept it because it violates Facebook policy, but you can edit it a little, then re-upload it, maybe it will be approved.

Your image is very unlikely to run ads, it will violate policy leading to account ban. You should hide that image
 
those sensitive images will violate facebook policy, i think you should blur it a bit
