Affiliate network list with contacts

Hi All,

For the latest project I'm involved with, I needed to compile a complete list of affiliate networks. What I did was to visit each site and include an address and phone (if any were available) and a direct link to the sign up page as well as a general link to the website. It's a pretty extensive list (certainly not all of them) but i thought all of you might be able to get some use out of this as well.
