It list out too many factors. what should I change?
 
Check the indexed pages of your site, if any of them give 404, redirect them to the homepage. It usually gives this error for this reason.

Also, if your site has any idle ad slots from the theme, remove them.

Indeed, if the OP does these minor changes he can get approval in next submission. Even though there are atleast 4 reasons mentioned, it is a generic reply that he has got from the adsense.

@online28 Fix the few things like content, title and navigation and you should be fine.
