Anyone knows what kind of situations account would triggered permanent disable? Is it relative to temp banded times or use bot?

Could be anything that IG thinks you are in violation of tos. Heck, your acc can be disabled right after you login after you have created.

The most common is overdoing your limits continously. Just test your accounts' limit and do it on safe settings.

Instagram. @OP, the majority of my accounts get banned because of super high settings. I always test their limits. Aside from that, I'm usually conservative with my other set of my accounts.
