    I'm seeing a URL ending in /404 showing up in the SERPs when I do a site:website.

    Any clue as to why this is showing up?

    You had a broken link that redirected to your 404 page and Google indexed that.

    1. Exclude your 404 page in your robots.txt file

    Code:
    User-agent: *
    Disallow: /404

    2. Login to Google webmaster tools and go to the remove url section, enter your 404 url.

    --> The page should be de-indexed in 24-48 hours from Google's index

    OPTIONAL:
    You can add the meta tag:
    Code:
    <META NAME="ROBOTS" CONTENT="NOINDEX, FOLLOW">
