step 1: reformat your hard drive (that means reinstall windows)
step 2: change your ftp passwords (and all of your other passwords also since they may have been logged)
step 3: contact your host and have them restore your website to a clean backup
I suggest you reformat your hard drive. You probably have a virus. Every time you login to your website, your username and password are logged and then your site is reinfected. This happened to me. Once you clean up your site google will re include it in the serps.
