same issues here, but as others have said, keep your actions really low. I think 300 is still too much, keep it below 200 likes and 200 follows a day, and less than 40 actions per hour. Warm up slowly.

this is all automatic mate, you need to wait until the ban lifts, it can take up to a month. You can´t contact instagram for this problems. You can keep using your account normally except following new people.

You need to research your hashtags, don´t reuse them and also you need to choose different type of reach for each hasthags, popular hashtags maybe over a millon post are pretty useless because there is just too much noise, choose hashtags below 100k post, then below 50k post, then below 10k post...
