As far as I remember some older version of Squid had limit of 128 simultaneous connections. That should be resolved in newer version, but please check squid config and doc to confirm it.

It depends from where you get you IP address. Public pools from ISPs can get a lot of bad rated IPs as some of users before where doing illegal or shady stuff. Try using some reliable mobile proxy provider as most of them are giving same public IP to thousands of mobile devices at the same time...
