Hi everyone,

My website gets around 7,000 daily visitors, all organic traffic from Google, and almost entirely from Germany. The site is in the medical/doctor niche.

I’m currently using Adsterra, but the CPMs feel very low for this volume of high-quality Tier-1 traffic, so I’m looking for better alternatives.

I’ve applied to Google AdSense twice, but both applications were rejected.

Which ad networks would you recommend for this kind of traffic? Any suggestions or personal experiences would be greatly appreciated.

Thanks in advance!

for german medical sites the adsense rejections are usually about the basics. you need a proper impressum and datenschutz page, a google certified consent banner (required for EU traffic) and original content with real author info, since health is a YMYL niche. I'd fix those and reapply before giving up on it. for alternatives I'd look at ezoic first, 7k a day is plenty for them, just ask if they have german advertiser demand. mediavine and raptive are mostly US focused so I'm not 100% sure how they treat german sites. with medical content, direct deals with pharmacy or telemedicine affiliates can also pay way more than display ads.

With 7,000 daily visitors, mostly from Germany, I think you have a good opportunity to improve monetization.

If I were in your position, I would focus on three things:

- Ad networks: Check Mediavine and Raptive's current eligibility requirements.
- Direct advertising: Approach relevant healthcare brands for direct ad partnerships.
- Affiliate marketing: Promote relevant medical products or healthcare services with proper disclosures.

I would also investigate why AdSense rejected the site twice before applying again.

My friend, you should definitely add ads. I wouldn’t use Google’s, though—they don’t pay the best. I’ve had good results with Mediavine and Raptive (formerly AdThrive).

With 7,000 daily visitors, mostly from Germany, you should definitely be able to get better monetization than Adsterra.

I'd recommend looking into these networks:

  • Ezoic – worth checking for smaller and medium-sized publishers.
  • Setupad – good option for European traffic and programmatic advertising.
  • Clickio – particularly interesting for European publishers.
  • Mediavine Journey – worth considering if your website meets their requirements.
Since you're in the medical niche, I'd also explore direct partnerships with healthcare companies, clinics, and medical service providers. Sometimes sponsored placements and relevant affiliate offers can generate considerably more revenue than display ads.

One important thing: with German traffic, make sure the network supports GDPR-compliant consent management and has strong demand from German advertisers.

I'd personally start by contacting Setupad and Clickio and asking them for an estimated RPM based on your traffic and niche.

Germany is considered Tier 1 country. I'll recommend you apply to all of them, Mediavine, Adthrive, Ezoic etc. Just all of them. Anyone that accepts you, you work with them.

Before switching networks, check your actual Page RPM and ad fill rate, not just CPM. With 7K daily visitors, low earnings could also come from poor ad viewability or unfilled impressions. Comparing these metrics will help identify whether the problem is the current ad rates or your ad setup.

I would first look at the reason behind the Ad Sense rejection rather than switching networks immediately. With 7,000 daily visitors, mostly from Germany, you have a decent opportunity to earn more from display advertising.

And I would test Setupad first, then compare it with Journey if the website meets its traffic and content requirements. Journey has a 1,000 premium-session minimum, but medical content receives more and avoid aggressive pop-under networks if you want to build a long-term medical authority website.

A couple of things I'd like to know:
  • Is the website publishing medical information, doctor profiles, or clinic reviews?
  • Are the 7,000 daily visitors generating how many monthly pageviews?
