For me, it’s fake traffic and manipulated metrics. A site can look great on firstlook, but once you check the traffic sources, relevance, indexing and actual link profile, the picture can be completely different. Finding publishers that are transparent, relevant and consistent is probably the biggest headache these days.

As far as i am concerned faked or deceptive traffic is the greatest concern the stats can look impressive yet the reality is another story i typically pay attention to traffic sources relevancy to my niches, and recency before placing on any site.
