Is this approach acceptable? Have any clients asked why you recommend services in other cities?

Adding multiple locations is not a good practice. In Google Local, you should add only one accurate location for each business.

You need multiple accounts for multiple areas in order to do this.

Yes its common for local SEO, just make sure each location page has unique useful content and not just copied city names.
