From my experience, the biggest mistake is targeting channels that look relevant on the surface but have a completely different audience than your offer. That's where most wasted spend comes from.
How important is language segmentation when targeting multiple regions?
Very important. I have seen campaigns get good CTR but poor conversions simply because the audience language did not properly match the ad and landing experience.
what metrics do you monitor to identify targeting inefficiencies before they become expensive
I mainly watch CPM, CTR, subscriber cost/CPA, and conversion rate. If CTR looks good but conversions are poor, it is usually a targeting issue rather than a creative issue.

For international campaigns, I would rather start with smaller, highly relevant channel groups and scale gradually than target too broadly from the beginning.
 
I think the biggest targeting mistakes is grouping too many countries into a single campaign .On Paper it looks efficient , but in reality a few countries often consumes most of the budget while delivering lower quality traffic.

language segmentation is also important even when users understand english , ads written in their native language often generate better engagement and higher conversions rate.

I pay close attention to CPM , CTR subscriber cost , conversion rate and retention quality after the click. A high CTR can sometimes be misleading if the traffic is not converting after joining a channel.
 
One thing I'd suggest is not assuming that regions with similar demographics will respond the same way. I've seen campaigns waste budget because broad geo targeting looked good on paper but user behavior was completely different. Language segmentation is also more important than many people think. Even when users understand a common language, localized messaging often performs much better.
