If you want something easy, Elementor or WPForms work well for building landing page forms. For more logic-based outputs, Tally or Typeform are great too. They let you set conditions without coding. Lots of YouTube guides out there that show full setups.

For a landing page with logic, Gravity Forms or WPForms are the easiest if you're on WordPress, both let you create forms that change based on what people type.
If you want something non-WordPress, Typeform is simple and looks clean.
For examples, just check landing pages from SaaS sites like Notion or Ahrefs, they’re good for inspiration.

Before 2022 i have used WPforms, but after that i tried typeform, but just becuase of pricing going higher and higer, i have moved to typeform alternative, and recently, i have using Quizify and Youform

if you need conditional logic and different outcomes based on user input then i would look at form builders that support multi step forms and dynamic fields. those features save a lot of custom development and make testing much easier later.
in case the data is dependent on answers to certtain questions having the ability to work with conditional logic nd calculation of fields becomes important. i would start by making a prototype of the survey nd only after that choose the platform to meet my needs.
