Three providers I've reliably had open SMTP + extra IPs + rDNS on a CentOS VPS without drama: Hetzner (open SMTP after a brief unblock request to support, very cheap for the IPs), Contabo (extra IPs are cheapest in market, but rDNS through ticket and they're slow to grant SMTP), and BuyVM (smaller but specifically SMTP-friendly out of the box, no negotiation). Avoid Hostwinds and most big-name budget hosts for PMTA — they say SMTP is allowed and then null-route you the moment you push real volume. One unintuitive thing: don't try to start with 5 IPs from day one. Get 1 IP, warm it for 2-3 weeks at low volume, then add the others. Cold-launching a PMTA setup with 5 dirty unwarmed IPs is the #1 way to get the whole subnet flagged in the first 48 hours.
