1) Will I get banned if I use adress that I used 6 years ago?
2) Will I get banned if I use IP adress I haven't used in a year?

1) Yes. Try to use a different address or to change existing address a little bit, like:
123 N Main St
123 North Main St
123 N Main Street
123 North Main Street
123 N Main St Suite 25

2) If that IP was banned Google will ban you again.
