Unfortunately you'll have to add them manually. It'll take a couple minutes, sorry. There's a quicker way if you're good with Excel.
You can use the & sign.
Say A1 is your proxy and port, and B1 is your username and password.
Just do this: =A1&B1
On the bright side, yes the proxy checker in UNF checks for all of those and also query the other data providers to make sure the proxies haven't been banned in the past.

Hey. If you wanted to do this even quicker, as well as have the txt file ready for future use you could try this... load the proxies into a text file and use the replace function in notepad.
So for example:
xxx.xxx.xxx.xxxx:8080
xx.xxx.xx.xxx:8080
Just click edit ---> replace
replace :8080
replace with :8080:username
Hope this helps. For me this is the easiest way to do this, plus my list is ready for any tool I may need.
