I have used CTR Theme on a few sites with good success. Just be sure the layout is compatable with Google's TOS.

I think this depends on what you want to do... Thesis can create some real nice looking sites, which is what I normally use, but then, if you want some straight up high CTR's you need to get some ugly ass themes lol.
