I recommend to all clients that when using a PBN, only use generic or partial match anchors. Branded, URL, and exact match are too easy for your competitors to find and report to Google. Save the Branded/URL anchors for (real) guest posts and niche edits.

For example if I choose to get 12 months post in health blog so topical trust flow will be health and along with that entire blog will be on health niche? OR just TTF will be health and site will be general? Can you share any sample?

The 'category' of the domain will give you an idea of how it's been set up and the 'theme' of the website. The TTF is just the type of links that are pointed at that domain. Generally, for example, a domain with many health TTF links will be in the health category.
