To say the least it is ugly but wondering how an Offline Consultant would advise a client like this Dentist on how to manage the bad PR she is getting.

I would find another dentist to help. Chances are these type of dentist/client will use the same legal tactic on all their dealings, including to the offline consultant themselves.
I always look out for red flags, what they do to others will often what they will do to you.
Plus, why not use your skills to help those who are worthy of your help?
More for their competitors.

Ahaha, yeah. She's really not thought this through. She now just comes across as extremely shady and out to rip people off. She's probably destroyed her business when all she had to do was hand the documents over to the guy so he could pass them on to his insurance company. Complete and utter madness.

Without knowing all that was in the confidentiality agreement I would assume that the law is one the dentist's side. Confidentiality agreements are not a breech of freedom of speech because no one is coerced to use them. This man could have gone to another dentist if he didn't feel right about it. He chose to stay with her because of the pain. She did not cause the pain, she fixed it. For a New York dentist $100 per day sounds reasonable when it comes to damages. It may have even been a clause in the contract, we don't know.
He broke the contract and now he is going to have to pay the consequences.
Those additional yelp reviews will not hold since it is very clear that they have never used her service.
Yes, she comes off as a bitch. I personally don't care if a dentist is a bitch. I care about if they get the job done well. With the patient in question she got the job done well. The problem was with how the office handled the paperwork.

Pretty sure the paperwork is part of "the job", so she didn't exactly do the job well.
And I think the issue people have is that if you're having patients sign such a contract, then you might be hiding something.
My dentist has never made me sign such a contract, and he has a 5 star rating on Yelp.

Maybe a valid defence would be that he was in distress and could not make reasonable decisions. I guess some judges, juries whatever would agree that not many people can think clearly when in pain.
It is also a question of local law whether emergency treatments can be tied to a willingness to waive stuff. Otherwise first thing in the E.R. would be to sign forms agreeing not to talk.

I think the big picture here is that most people don't like to do business with a SHADY person like this. I mean, if you're offering a legit service, why would you be worried about people leaving you negative reviews?
Unless the person is a psychopath, I've never heard of a customer/patient/client leaving a bad review when they were pleased with the service they received. Makes no sense at all.
I do realize in some cases a business owner does need to cover their own ass, but prohibiting the customer to leave reviews? Sounds shady.

With yelp goodluck with trying to talk to a human. And when you do it's the same canned speach. You really have to be creative with yelp.
With yelp I go in steps.
1. White hat- contact the bad reviewer and try to work it out.
2. Grey hat- Contact yelp if you can and scream until they comply with taking off the add with above mentioned steps.
3. Black hat- ?
time to play the game.
