Copeac, Neverblue, ROI Rocket, Hydra, FluxAds, CoProsper, that's just a few. From what I've been told, it's best not to mess with Azoogle and MaxBounty when using black hat CPA tactics. They use their own, proprietary back end software and most of their owners are some pretty sharp guys. Save those two networks for your legitimate traffic
You don't have to have a website setup, but it definitely helps. If they ask about incentivized offers, just say no. You can always use them later on down the road anyway.
The best thing you can do is apply, and then call them up on the phone within 24 hours. Tell them you're a PPC marketer and that you've had success in the past with (insert other CPA network name here) but that you'd like to give them a try. Tell them you've had success with email submits (free iphone kinda stuff), payday loans, ringtones, education offers, stuff like that.
