Youtube Video Issue

Hi everyone,
I am new and kind of confused that in which section this question should be.
I am looking for the suggestions that some one has uploaded my video on youtube without my permission. I tried to register my copyright infringment notice to youtube but it did not work. Can anybody give me suggestions/guidance that how to remove (proper way or tricks) that video.

Any suggestion and comment will be helpful.

well, because it is your video there are guys on fiverr who will flag it 30-50 times. are you sure its only uploaded once?
