How much time does it take to approve by a program?
How will get to know that some program has accepted me?

a) In your publisher home page, you can see a link on the lower right corner under "Today's Tasks" -- "Review xx pending application(s)."
Else, you can click on "Get Links" - "By Relationships" - "Pending Applications"
b) Some could take a day, some could take forever ..... 
It really depends on the individual program
c) Usually, you will receive a welcome email, if not, just check on your list of approved advertiser

No kidding on the "forever." I have about 8 pending applications that have been sitting for 6 weeks now. Fortunately, none of them are programs that I feel like I really need.
I've always gotten an e-mail directly from the advertiser letting me know that I've been approved. Make sure that your e-mail settings are correct...it's pretty intuitive so you should be able to figure it out pretty easily by looking at it.

you will not know the criteria for sure.
if you really want that program badly, then email the affiliate manager listed in the program detail page.
if you are not within US, many will be declined by default, but that does not means it's not possible to get approve.
i have a US financial program which decline me by default, then i wrote to the affiliate manager.
what she really wants is that i make sure that my traffic is for the US and I have a website promoting the program.
once you have that, then you are likely to be approve.
but that said, some will decline you no matter what, if you are not in US
