well first off the listing on the serp page is an adwords ad. So to get the redirect from the ad on the serp page you need to use a different display url and destination url when you set up your ad.
The destination url is then doing a meta refresh redirect to the hoplink url.

Now the Adwords rule regarding the display url is that it must match the destination url. The exception to this rule is if your destination url is a custom tracking url (which is what they're doing in this case). Now this is only OK with adwords if the tracking url then redirects to the actual url displayed in the ad.

So this setup is OK with adwords
display: www.example.com >> destination w/ custom tracking:
www.trackingurl.com/{customstuff} >> landing page: www.example.com

But this setup is NOT OK with adwords
display: www.example.com >> destination w/custom tracking:
www.trackingurl.com/{customstuff} >> landing page: www.salespage.com

Well in the example you gave they are obviously doing the 'NOT OK' method because when you click on the ad you get sent to the tracking url which then does a meta refresh and redirects you to the clickbank salespage instead of the url displayed in the ad.

There's a few ways this could be accomplished. The easiest is to just set it up in the 'OK' fashion, redirect to the tracking url which then redirects to the display url. Submit the ad and then after it gets approved change the redirect to the salespage.com (via hoplink) landing page.

Now if you do that though you'll probably get caught fairly quickly (google isn't stupid and they watch for that). So the normal way to do it is after the ad is approved when you change the redirect you'll use htaccess (or a php file) to determine who the visitor is, if it's a visitor from an adwords or google ip then send them to a page they approved that redirects to the display url, if it's an ip from anywhere else then send them to the page that redirects to the hoplink url. Google sees your link as OK, but your target visitors go to your salespage offer instead.

@OP - OK now that you understand how it's done you should edit your first post to remove the screenshot and the domain name. It's a pretty cool trick if you can pull it off, but definitely blackhat and you're outing the marketer in this thread. If google sees what he's doing it could get his ad deleted and possibly get his account banned.

The OP wasn't asking about whether its ok to run a ppc campaign to your affiliate site. He was wondering how the ad was able to have a display url that was different than the destination url, and how it was then further getting redirected to the affiliate site without ever going through the displayed url at all.

The displayed url in that particular ad doesn't even exist, it's only use is to make people think they'll end up on a trusted site subdomain. It's a trick to gain trust so that more people will click the ad. Pretty slick really, but a separate issue than just whether it's ok to run a campaign to your affiliate site.

No I didn't see anything to indicate what he's using, but I only did a cursory look at the page source.

If you follow the destination url it just goes to a standard html page with a metarefresh redirecting to the hoplink. If you just type in the domain root then it redirects to the login page you listed. He has it set up to appear to be a clickbank login page, but I wouldn't advise typing in your username and password there, lol. I didn't pay a lot of attention to it, but if you look at the source of that login page its just a standard form post to input login and password and then a submit button.

OK I just went back and looked at the source on that login page again. The page is also using a javascript files 'mootools-1.2.4-core-yc.js' and 'mootools-1.2.4.4-more.js'. I didn't bother to look at those files so I'm not sure if he's using that for the tracking or for something else. There's really not enough in the source to tell what exactly he's doing. If you view the source on the page and then view those mootools js files then you might be able to find out more.

Really though for what the guy is doing the tracking script doesn't matter. Since the visitors pass through his website before being sent to the hoplink the guy could even just use cpanel and view his server logs and get some basic tracking info that way. Basically the method I explained just accomplishes getting the display url of the ad to be different than the true destination.
