The website you gave as an example probably uses a CMS ( google it ). If you want to design an website from scratch you have to start with the design(Photoshop) and then code it using css and html.

Depends on what you mean by design?
Design can mean anything like
- code
- "draw"
But I assume you mean something more like
create.
Your best bet is to start with Wordpress. Look
for some free themes within the wordpress
theme area once you have uploaded the frame-
work to your site.
If that still doesn't work, I'd check out a site
like http://themeforest.net/ a lot of the themes
are available for free download here on BHW
but if you buy the theme there you might get
support in setting it up, and it's the right thing
to do (if you don't have the money, start with
the free download and once you make a bit, go
and buy the theme).

Learn the basics first before you could go for a high end one. Video samples can be helpful for you.
