Default Tax Forms For Canadians?

Hey guys, im looking for a response from fellow canadians, ok im filling out my linkshare tax info but since im from Canada what Tax Form do i need to request? And after i fill out all this information what do i need to do with it and what do i have to give my accountant who does my taxes?

What exactly are they asking for? I've been doing aff marketing for years and years and I have never had to give a company any sort of tax info. If they ever require some sort of US tax id I just put "n/a", and I have never had any problem.

BTW HAPPY CANADA DAY MOFO!!!

linkshare says u have to fillout tax info before u can receive payment and happy canadas day!

Simply go to revenue canada and pick up a general tax return for the year 2008.Have your accountant fill it out with the linkshare portion and your total income entered on line 150 of your return and fax them(linkshare) over a copy.Just dont send it in to Revenue Canada.
