Re: Business Name
If the form is for tax info, you may get in trouble since it's the IRS you're technically messing with. That said, the affiliate network may not cross reference the form with ones they've gotten in the past.
If the form is just for the affiliate network, then nah, worst case scenario is you've broken their ToS and may get banned but you've done nothing illegal.
Just to let you know, I've actually applied for affiliate networks with my legal name twice; once as me, once as my company, with no problems.

The tax info should be for my business. Its asking for a first and last name as well as company info.
Actually its just for contact info

Re: Business Name
I'd say it's safe to just put your real name.
If you still feel a bit wary, just put in a nickname or something;
like if your name is James, put in Jim even if you don't normally go by Jim.

Well, not to toot my own horn here but what you do is incorporate and appoint anyone you like as the business agent for your corporation. Get an EIN for the corporation and set up a corporate bank account. Use whatever phone you want for the corporate contact and you are anonymous.

I have an EIN and a bank account under my assumed name. Can I still do this with just a DBA.

Are you referring to your DBA name or did you just make up an identity?
Not clear on this.
