So I had this great idea for a blog, it has quite low competition and main keyword gets 110,000 searches every year. But main searches come from Italy and Germany. Should I try to make it? Would be my first try with anything like that.

If anything I'd just throw AdSense on there to see what advertisers keep advertising on there after like a month, then see if you can go direct. Let Google handle the targeting for you out of the gate if you don't know what direction to go yourself with it.

You should try it. Click from germany and italy are not that bad. If you have good traffic you will get the clicks and revenue also
